First-Time Patient?

REGISTRATION

Please come about 15 minutes early for your first appointment. Our office is paperless and digital. All patient information is stored on our computers. You will enter all patient information on a electronic tablet using a inkless pen.

PARKING & DIRECTIONS

You are free to park in front of or behind our building.  Address, Hours, Phone Numbers, and Directions to Immanuel Dental.

PLEASE BRING THE FOLLOWING INFORMATION:

  • Current medical doctor/clinic’s name, address, and phone number
  • Previous dental office name, address, and phone number
  • A list of current medications and doses
  • Any pertinent medical and dental information that will help us in giving you the best possible care and service

CURRENT DENTAL X–RAYS

Before your first visit we will need a copy of any X-rays that were taken in the past 12 months (Bitewings, about 2-4 films) or the last 3-5 years (Full Mouth Series, about 18-20 films). If you are unsure of the status of your last X-rays, please contact your previous dental office or your current dental insurance company, they will be able to tell you the last time you had them taken. We will need to the X-rays to make an accurate diagnosis of your dental health. We can also assist you in getting previous dental information from your previous office if you would like. You will need to come to our office to sign a release form that ensures your previous office that you have requested this information.

DENTAL INSURANCE

Please bring your current dental insurance card. If you are insured under someone else's insurance, please bring that person's social security number, the employer the insurance is through, and the group number. If you have a additional dental insurance coverage, please bring the same information requested above.

DO YOU NEED PRE–MEDICATION FOR YOUR DENTAL APPOINTMENT?

There are times when our patients need to take antibiotics prior to a dental visit. The main two reasons are related to your heart or prosthetic replacements involving joints (i.e. hip or knee). If you are unsure, please contact your medical doctor, or please call us. If you are in need of pre–medication but you have ran out of your prescription, we can order it for you provided you know the name and the dose of your prescription. Otherwise, we will follow the guidelines given by the American Heart Association.
